Health and care organisations in Hackney and City want to hear your views to shape the future of the local integrated health and care system.

This focus group will look at:

· What is important to you in health and care?

· What works well now?

· What change would you like to see in the next few years or even 10?

· What does it mean for health and care to be joined up?

Your views will be used to develop an outcomes framework for the Integrated Commissioning programme in Hackney and City. This will be used to show what residents value and measure progress towards achieving this.

We are keen to hear from a diverse mix of Hackney and City residents including:

· Residents who are 50+

· Black and ethnic minority groups including migrants and refugees

· Young people under 30

· People in work

Food and refreshments will be provided. Please arrive at 5.15pm to allow time for registration
